   I confirm that the information provided is accurate to the best of my knowledge. I give consent for Salik Foundation UK to store this information securely and use it to assess my (or the referred individual's) eligibility for support.

2. Confidentiality & Data Protection
   I understand that all information will be kept confidential in line with the Data Protection Act 2018 and GDPR, and will only be shared with third parties where necessary for support services, and only with my explicit consent or where required by law.

3. Voluntary Participation
   I understand that engagement with Salik Foundation's support is voluntary and does not replace medical or clinical treatment. I may withdraw consent or support at any time.

4. Safe Conduct
   I agree to treat staff, volunteers, and other service users with respect and to adhere to the charity’s policies for respectful, safe engagement.

5. Referrer Declaration (if applicable)
   If I am making this referral on behalf of someone else, I confirm I have their permission to share this information and that they understand how it will be used.
